Four workers killed at an oil refinery in Wales

The BBC news website has reported that four workers were killed in an explosion and fire at an oil refinery in Wales last week.

The police and Health and Safety Executive are investigating the tragic accident but has confirmed that it is too early to speculate about the cause of the explosion.
