Why do road traffic accidents happen?

The Telegraph newspaper has reported that the Institute of Advanced Motorists (“IAM”) has produced a report on the causes of road traffic accidents.
The IAM considered 700,000 accidents which took place between 2005 and 2009. The IAM found that the main cause of fatal accidents is “driving error or reaction”. Other causes include travelling too fast, careless driving, inexperience and distractions whilst driving.
The report found that age is a factor, older drivers tend to fail to look properly and younger drivers are more likely to drive too fast.
